Is the bulb included? No. It requires four E26 medium base bulbs (A19 shape), max 100W each, sold separately.

Is it dimmable, and what dimmer should I use? Yes, it is dimmable. Use a dimmer rated for the bulb you install.

Can I install this in a bathroom or outdoors? It is damp location rated, so bathrooms and laundry rooms are fine; it is not rated for uncovered outdoor exposure.
