Is the bulb included? No. It requires six E12 bulbs, max 60W each, sold separately.

Is it dimmable, and what dimmer should I use? Yes, it is dimmable. Use a compatible dimmer and dimmable bulbs; the manufacturer does not specify a dimmer brand.
