Is the bulb included? No, bulb is not included. Requires one E26 base bulb up to 100W.

Is it dimmable, and what dimmer should I use? Yes, it is dimmable. Use a standard line-voltage incandescent/halogen dimmer. LED bulbs must be dimmable for full range.

Can I install this in a bathroom or outdoors? It is rated for damp locations, so it can be used in bathrooms (away from direct water spray) or covered patios. Not for wet locations.
